APPLICATION FOR NEW CONNECTION
Requirements
For Permanent House Connection:
• Electrical Plan
• Electrical Permit
• Certificate of Final Inspection & Completion
• Certificate of Occupancy
• Payment of P300 service fee
• Payment of 1 month Bill deposit (based on the
electrical plan)
• Signing of Contract (P50.00 Notarial Fee)
Temporary Connection for Bunk Houses:
• Electrical Plan
• Permit for temporary house connection
Process Flow
⚈ Attend Pre-Membership Education Seminar (PMES) for those who are not yet a member
Schedule for PMES:
Tuesday, 8:00AM - 12:00NN (via ZOOM)
Friday, 8:00AM - 12:00NN (At Main Office)
⚈ Secure all requirements and documents from your Local Government Unit
• Provide drawing of location of map
• Electrical Lay-out Plan signed by PEE.
• Electrical Permit
• Certificate of final electrical inspection (Signed by the Municipal Engineer/BFP/PEE --- Original Copy)
• Photocopy of valid ID (any government issued ID)
⚈ Submit requirements to Institutional Service Department(ISD) office for evaluation
• Refer to an Electrical practitioner or to your nearest Barangay Electrician.
• Please install the service entrance based on your electrical plan. So that when the inspector comes, it is already prepared.
⚈ ISD will schedule for the Service Entrance inspection
• The inspector will check the specifications and sizes used based on your submitted electrical plan.
⚈ If approved, proceed to ISD office for payment and signing of contract.
• Service fee of P300
• Notarial fee of P50
• 1 month bill deposit based on the load of your electrical plan
* If not approved, comply all necessary improvements. ISD will schedule house wiring inspection again.
⚈ Issuance of Connection Order
⚈ Forward Connection Order to Technical Service Department(TSD) office for execution of servicing
